Agreement to Terms

By accessing or using the website, mobile application, or services of Innovative Physical Therapy Institute ("we," "our," or "us"), you agree to be bound by these Terms of Service. Description of Services

Innovative Physical Therapy Institute provides mobile concierge physical therapy services, including but not limited to:

• Geriatric movement and fall prevention programs
• Pain science and nervous system reset therapy
• Lymphedema and edema wellness management
• Non-invasive Tissue Regeneration & Pain Reset Program (utilizing Shockwave Therapy)
• In-home, office, or gym-based physical therapy sessions

All services are provided by licensed physical therapists and are delivered at a location convenient to the patient, including homes, offices, gyms, and other appropriate settings within our service area.

Cash-Pay / Private-Pay Model

All services provided by Innovative Physical Therapy Institute are private pay (cash-based) only. We do not accept insurance, participate in insurance networks, or bill insurance companies on behalf of patients.

By scheduling an appointment or receiving services, you acknowledge and agree that:

• You are responsible for the full cost of all services at the time of service or as otherwise agreed upon
• We do not bill insurance companies and are not responsible for any insurance reimbursement
• Upon request, we can provide a detailed superbill that you may submit to your insurance company for potential reimbursement; however, reimbursement is not guaranteed and depends on your individual plan
• Pricing for all services will be provided transparently prior to the commencement of treatment

Appointment Scheduling and Cancellation

Appointments may be scheduled through our website, mobile application, phone, or email. By scheduling an appointment, you agree to the following:

• Cancellation Policy: We require at least 24 hours' notice for cancellations or rescheduling. Late cancellations or no-shows may be subject to a cancellation fee.
• Late Arrivals: If you are not available at the scheduled time and location, your session may be shortened or rescheduled at the therapist's discretion.
• Provider Cancellation: In the event that we need to cancel or reschedule your appointment, we will provide as much advance notice as possible and work with you to find a suitable alternative time.

Patient Responsibilities

As a patient, you agree to:

• Provide accurate and complete health information necessary for your treatment
• Inform your therapist of any changes in your health condition
• Follow your prescribed treatment plan and home exercise program to the best of your ability
• Provide a safe and appropriate environment for in-home or on-site treatment sessions
• Make timely payments for services rendered
• Communicate openly with your therapist about your comfort level, pain, and progress

Informed Consent

Before beginning treatment, you will be asked to provide informed consent for physical therapy services. This consent will outline the nature of the treatment, potential risks and benefits, alternative treatment options, and your right to refuse or discontinue treatment at any time. Physical therapy involves physical contact and therapeutic exercises; your therapist will explain all procedures before performing them and will obtain your verbal consent throughout the course of care.

Limitation of Liability

While we strive to provide the highest quality of care, physical therapy involves inherent risks. By using our services, you acknowledge that:

• Results may vary and are not guaranteed
• Physical therapy may involve temporary discomfort or soreness
• We are not liable for any injury or damage that occurs outside of our direct care
• Our liability is limited to the extent permitted by applicable law

Website and Mobile Application Use

By using our website and mobile application, you agree to the following:

• Accuracy of Information: While we strive to provide accurate and up-to-date information, we make no warranties regarding the completeness or accuracy of information on our website or application.
• Not Medical Advice: Information provided on our website and application is for general informational purposes only and should not be considered a substitute for professional medical advice, diagnosis, or treatment.
